Authorized users are able to make changes to account and receive account information.

Maquoketa Valley Electric Cooperative (MVEC) is committed to completing your project in a timely and professional manner. MVEC incurs significant expense to extend fiber to each service location and as such, only plans on completing service drops at this time to members wanting internet and/or telephone service. MVEC and its contractors will need to gain access to the service address listed above in order to install facilities.

By signing below, I am authorizing MVEC to install a service drop at the service address listed above. By authorizing the service drop, I am agreeing to receive internet and/or telephone service from MVEC. I also agree that failure to purchase service after the drop is installed will result in MVEC billing me the full cost of the service drop.

Furthermore, I understand that I am responsible for identifying member-owned underground facilities that are not marked or protected by Iowa One Call. This includes secondary electric, water, phone, gas, septic, well head and any other underground facilities. I agree that MVEC and its contractors are not responsible for damage to any unmarked, member-owned underground facilities as it relates to installation work at the service location listed above.

I also understand that despite MVEC’s most diligent efforts, yard ruts or other cosmetic property damage may occur as MVEC and/or its contractors move vehicles or equipment onto the property. MVEC and its contractors will make every reasonable effort to repair/restore property before completing work at the service address above.

Your delay in returning this signed agreement will delay the completion of this work and related services. Please read, complete and return in order to begin work in a timely manner. If you have specific questions or concerns, please contact MVEC at 800-927-6068 or email fiber@mvec.coop.
